1. Increasing social, community and recreational participation
We work with you to increase your community participation and engagement by providing supports that enable you to:
- Learn new skills – or to enhance the abilities you already have.
- Stay in touch with your family, friends and other important people in your life, and meet new friends in your local community
2. In-home support, daily living activities and domestic duties
We provide personalised disability assistance in the form of in-home care supportive services like meal preparation, transportation and housekeeping or assistance with hygiene and personal care to enable you to live independently in your own home for longer. If your needs change, we will work with you to make sure you get the right level of support when you need it.
3. Support coordination
At TLC we want to make sure that the goals you set in your plan actually happen! This might mean that you need some extra support to put some, or all, of your plan into action. Our skilled, experienced and qualified support coordinators work creatively and resourcefully with you to ensure you use your support budget in the most effective way possible to achieve your goals.
4. NDIS plan management
The importance of managing your NDIS plan cannot be understated. That’s why we work with you to ensure your funds are allocated correctly and reduce your risk of running out of funds prematurely. We help you make the most from your plan, without wasting money and making sure that you’re getting the support and care you need.
